Student Travel Request Form
Student Travel Request forms are required for any campus-sponsored event where students are leaving campus. Please complete and submit the Student Travel Request form at least 2 business days prior to your travel date. Completed student travel request forms should be submitted to the Office of the Vice President for Student Affairs (VPSA) at studentaffairs@wou.edu.
There are TWO different forms based on the type of travel that will be taking place – Mandatory Travel Events, or Optional Travel Events. Examples of Mandatory Travel Events may include class activities (i.e. required field trips) or training required for employment that involves travel. Please see below for the different forms:
Mandatory Student Travel Request Form (PDF Fillable)
A completed form includes:
- Travel information, transportation details, and travel approval signatures; and
- Signed Emergency Contact Person information for each traveler.
Optional Student Travel Request Form (PDF Fillable)
A completed form includes:
- Travel information, transportation details, and travel approval signatures;
- Signed Student Liability Waiver form for each traveler; and
- Signed Emergency Contact Person information for each traveler.
The Vice President for Student Affairs reviews student travel requests. If approved, a signed copy of the student travel request form (including liability form and emergency contact form) will be emailed to the contact person, advisor/department head, *ASWOU office coordinator, and Campus Public Safety.
If disapproved, a signed copy of the student travel request form will be emailed to the contact person and advisor/department head, and *ASWOU office coordinator.
*If traveling group is a funded ASWOU student club or organization.
Contact person should ensure a copy of the Emergency Contact Person information sheet is in each traveling vehicle prior to departure.
All student travel must adhere to the university Student Travel policy (ASA-04-017). If you have questions about this policy, contact VPSA at 503-838-8221 or studentaffairs@wou.edu.
Driver Authorization Forms
If you are driving a state vehicle, personal vehicle, or van (12 or 15 passenger), you must complete all required driver authorization forms and submit them to Campus Public Safety prior to your travel. It is highly advised that you submit all required driver authorization forms at least five days before your travel dates. Failure to submit required forms may result in a disapproved travel. Campus Public Safety will notify drivers if they are approve or disapproved drivers under University sponsored business.
