Please fill out this form so our staff can review your request and find a time for you to pick up your equipment. There are also paper forms available in the VRC (WUC108). Items are subject to availability, we utilize a wait list when needed. Be advised that we will periodically wipe the device’s memory clean, so be sure to backup any files you create on them to either cloud-based or external storage.
